Absence Form

If, for any reason, a student cannot attend a session, the absence form must be filled out with accurate and detailed information concerning the reason for the absence.  This is mandatory and must be followed to ensure we have the correct measures in place to protect and safeguard each student.

You will receive a confirmation email once the form has been filled out for your records.
